Effective Date: January 1, 2026
1. Participation Acknowledgment
I, the undersigned, acknowledge that I have voluntarily chosen to participate in physical training, athletic evaluations, and/or exercise programs (the “Programs”) offered by Sable Sports Group. I understand that the Programs may involve high-intensity physical activity, including but not limited to strength training, cardiovascular exercise, agility drills, and sport-specific movements.
2. Assumption of Risk
I am aware that physical training involves inherent risks, including but not limited to:
- Muscle strains, pulls, or tears.
- Broken bones, joint injuries, or concussions.
- Cardiovascular complications, including heart attack or stroke.
- Injuries resulting from equipment failure or improper use of facilities. I voluntarily assume all risks associated with my participation in these Programs.
3. Medical Representation
I represent that I am in good physical condition and have no medical reason or impairment that would prevent me from participating in the Programs. I acknowledge that Sable Sports Group recommends that all participants consult a physician before beginning any new exercise regimen. If I have any pre-existing conditions, I have disclosed them to the trainer or will do so prior to beginning activity.
4. Release and Indemnification
In consideration of being allowed to participate, I, for myself and my heirs, hereby release, waive, and discharge Sable Sports Group, its owners, officers, employees, and independent contractors from any and all claims, liabilities, or causes of action arising out of any injury, loss, or damage to my person or property, even if caused by the ordinary negligence of the Company.
5. Emergency Medical Treatment
I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during the Programs. I agree to be financially responsible for any costs incurred as a result of such treatment.
6. Media Release
I grant Sable Sports Group permission to use photographs or video footage of my participation in training for promotional purposes (social media, website, etc.) unless I revoke this permission in writing.
7. Governing Law
This Waiver and Release shall be governed by and interpreted in accordance with the laws of the State of New York.
